Accident prevention across the high risk industries is currently impacted by limitations in accident theory and methodologies. As a result, reductions in injuries and fatalities in most domains are plateauing. The aim of this research is to reconcile the gaps in knowledge and address the limitations associated with existing accident causation theory and methodologies. The outputs of the project will include a new theory of accident causation, a new proactive risk assessment methodology and a new methodology for analysing accidents. This will provide organisations and researchers with a powerful framework which will enhance accident analysis and prevention activities. The impact will be new reductions in accidents, injuries and fatalities.
